{"id":26715,"date":"2012-04-24T09:04:37","date_gmt":"2012-04-24T13:04:37","guid":{"rendered":"http:\/\/asweetlife.org\/?p=26715"},"modified":"2015-12-29T08:38:37","modified_gmt":"2015-12-29T13:38:37","slug":"the-significance-of-the-victoza-product-label-update","status":"publish","type":"post","link":"https:\/\/asweetlife.org\/?p=26715","title":{"rendered":"The Significance of the Victoza Product Label Update"},"content":{"rendered":"<p><a href=\"https:\/\/asweetlife.org\/wp-content\/uploads\/2012\/04\/VictozaPen.jpg\" rel=\"mfp\"><img decoding=\"async\" class=\"aligncenter size-full wp-image-26729 lazyload\" title=\"VictozaPen\" src=\"data:image\/gif;base64,R0lGODlhAQABAAAAACH5BAEKAAEALAAAAAABAAEAAAICTAEAOw==\" data-src=\"https:\/\/asweetlife.org\/wp-content\/uploads\/2012\/04\/VictozaPen.jpg\" alt=\"\" width=\"565\" height=\"165\" data-sizes=\"auto\" data-srcset=\"https:\/\/asweetlife.org\/wp-content\/uploads\/2012\/04\/VictozaPen.jpg 565w, https:\/\/asweetlife.org\/wp-content\/uploads\/2012\/04\/VictozaPen-300x88.jpg 300w\" sizes=\"(max-width: 565px) 100vw, 565px\" \/><\/a><\/p>\n<p style=\"text-align: justify;\">Novo Nordisk&#8217;s widely popular GLP-1 analog <a href=\"http:\/\/www.victoza.com\/\" target=\"_blank\">Victoza<\/a> made headlines at the beginning of the year when celebrity chef <a href=\"https:\/\/asweetlife.org\/jessica-apple\/blogs\/type-2-blogs\/is-paula-deen-for-real\/23906\/\" target=\"_blank\">Paula Deen became a spokesperson for the drug<\/a>. \u00a0This month Victoza is in the news again after \u00a0Novo Nordisk received approval from the U.S. Food and Drug Administration (FDA) to update the product label for Victoza\u00a0(liraglutide). \u00a0Victoza product label update? \u00a0Is that really something significant? \u00a0In this case, yes.<\/p>\n<p style=\"text-align: justify;\">The Victoza product label update shows data indicating Victoza&#8217;s superior blood sugar control when compared<strong>\u00a0<\/strong>to Merck&#8217;s Januvia\u00a0(sitagliptin) in a study where both\u00a0products were taken in combination with metformin in adults with type 2 diabetes. \u00a0Victoza also showed greater weight reduction when compared to Januvia. \u00a0But perhaps more significantly, the update\u00a0also includes data demonstrating the safety and efficacy of adding basal insulin to Victoza\u00a0and metformin for the treatment of adults with type 2 diabetes. \u00a0I had the opportunity to ask Jason Brett, M.D., Sr. Medical Director, Novo Nordisk, about the Victoza product label update and what it means.<\/p>\n<p style=\"text-align: justify;\"><em><strong><a href=\"https:\/\/asweetlife.org\/wp-content\/uploads\/2012\/04\/Jason-Brett-M.D.jpg\" rel=\"mfp\"><img decoding=\"async\" class=\"alignright wp-image-26734 lazyload\" title=\"Jason Brett, M.D\" src=\"data:image\/gif;base64,R0lGODlhAQABAAAAACH5BAEKAAEALAAAAAABAAEAAAICTAEAOw==\" data-src=\"https:\/\/asweetlife.org\/wp-content\/uploads\/2012\/04\/Jason-Brett-M.D-251x300.jpg\" alt=\"\" width=\"201\" height=\"240\" \/><\/a>The Victoza product\u00a0<\/strong>l<strong>abel update indicates that Victoza<\/strong><strong>\u00a0shows greater efficacy when compared to Januvia. Can you tell us a little bit about the study that proved greater efficacy? \u00a0Can you explain the significance of this?\u00a0<\/strong><\/em><\/p>\n<p style=\"text-align: justify;\">The Victoza\u00a0vs. Januvia\u00a0study was a 26-week randomized, open-label trial in people with type 2 diabetes which found that Victoza<sup>\u00a0<\/sup>demonstrated superior blood sugar control when compared to Januvia<sup>\u00a0<\/sup>as well as greater weight loss (a secondary endpoint). Specifically:<\/p>\n<ul style=\"text-align: justify;\">\n<li>Patients treated with 1.2 mg and 1.8 mg of Victoza\u00a0experienced greater reductions in A1C than those treated with Januvia\u00a0100 mg tablets, all in combination with metformin (-1.2% and -1.5% versus -0.9%).<\/li>\n<li>Victoza\u00a0provided greater weight loss versus patients treated with Januvia\u00a0(2.7 kg [5.94 lbs] and 3.3 kg [7.26 lbs] for 1.2 mg and 1.8 mg respectively, 0.8 kg [1.76 lbs] for Januvia).<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">These results are significant because Victoza\u00a0represents an effective option for people with type 2 diabetes who are struggling to lower their blood sugar. The expanded label should provide physicians with greater confidence to prescribe Victoza\u00a0to patients with type 2 diabetes needing better blood sugar control.<\/p>\n<p style=\"text-align: justify;\"><em><strong>Why is a GLP-1 analog (Victoza<\/strong><strong>) being compared to Januvia, a\u00a0<\/strong><strong>DPP-4 inhibitor? \u00a0Would it be more relevant to have a comparison of Victoza<\/strong><strong>\u00a0and Byetta<\/strong><strong>?<\/strong><\/em><\/p>\n<p style=\"text-align: justify;\">Although Januvia is not a GLP-1 receptor agonist like Victoza\u00a0and Byetta, as a DPP-4 inhibitor, it is an incretin-based therapy. Victoza\u00a0has been compared to Byetta. Head-to-head clinical studies have demonstrated that Victoza\u00a0provides significantly better A1C reduction than Byetta. Specifically:<\/p>\n<ul style=\"text-align: justify;\">\n<li>The LEAD-6 trial, published in 2009 in <em>The Lancet<\/em>, \u00a0found that treatment with Victoza\u00a0led to a statistically significantly greater drop in A1C of 1.12% compared to 0.79% in the Byetta\u00a0group, and Victoza\u00a0was also significantly better than Byetta\u00a0at lowering fasting plasma glucose -29 mg\/dL vs\u00a0 -11 mg\/dL).<\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><em><strong>Do you expect to see more doctors prescribing Victoza<\/strong><strong>\u00a0over Januvia<\/strong>\u00a0<strong>based on this label change?<\/strong><\/em><\/p>\n<p style=\"text-align: justify;\">Novo Nordisk cannot speculate on physicians\u2019 prescribing behaviors, but we are excited that the expanded label includes data that both demonstrate that Victoza\u00a0provides superior blood sugar control when compared to Januvia\u00a0and that Victoria\u00a0in combination with basal insulin is a safe and effective treatment for type 2 diabetes.<\/p>\n<p style=\"text-align: justify;\">This expanded label means that a broader range of type 2 diabetes patients may benefit from Victoza.<\/p>\n<p style=\"text-align: justify;\"><em><strong>Victoza<\/strong>\u00a0<strong>showed greater efficacy when compared to Januvia<\/strong><strong>, but what about side effects? \u00a0<\/strong><\/em><\/p>\n<p style=\"text-align: justify;\">The 26-week, open label study comparing the efficacy and safety of Victoza\u00a0with Januvia, found that the adverse reactions reported in equal or greater than 5% of the patients treated with Victoza\u00a0were nausea (23.9% vs. 4.6%), headache (10.3% vs. 10.0%), diarrhea (9.3% vs. 4.6%) and vomiting (8.7% vs. 4.1%).<\/p>\n<p style=\"text-align: justify;\"><strong>Moving onto questions about using Victoza with basal insulin, has Victoza<\/strong><strong>\u00a0been approved for use only with Levemir, or with other basal insulins as well?\u00a0<\/strong><\/p>\n<p style=\"text-align: justify;\">The FDA has approved the Victoza\u00a0label to include the approval of basal insulin as an adjunct therapy to Victoza\u00a0for the treatment of type 2 diabetes. \u00a0While the trial on which the Victoza\u00a0label expansion for use with insulin was conducted with Levemir, the label expansion means that it can be used safely with any basal insulin.<\/p>\n<p style=\"text-align: justify;\"><em><strong>What studies were done to demonstrate the safety of adding basal insulin to Victoza<\/strong><strong>?<\/strong><\/em><\/p>\n<p style=\"text-align: justify;\">The expanded label is based on a randomized, open-label study designed to establish the efficacy and safety of using Victoza<sup>\u00a0<\/sup>in combination with Levemir, a treatment approach that may benefit patients later in the treatment paradigm who are struggling with blood sugar control.\u00a0 The study found that:<\/p>\n<ul style=\"text-align: justify;\">\n<li>After completing 12 weeks of treatment with Victoza\u00a0and metformin, 50% of patients reached the ADA target for blood sugar control (A1C &lt;7%).<\/li>\n<li>After an additional 26 weeks, patients randomized to add on Levemir\u00a0once a day to their Victoza\u00a0and metformin regimen had further A1C reductions of -0.5%.\n<ul>\n<li>Furthermore, 43% of patients in the Victoza, metformin and Levemir\u00a0group reached the ADA target for blood sugar control (A1C &lt;7%) at 26 weeks vs. an additional 17% in the Victoza\u00a0and metformin group.<\/li>\n<li>Patients did not gain weight after Levemir was added to their treatment regimen.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><em><strong>How great is the risk for hypoglycemia when adding basal insulin to Victoza<\/strong><strong>?\u00a0<\/strong><\/em><\/p>\n<p>Based on the results of this trial, the risk of hypoglycemia when adding basal insulin to Victoza\u00a0appears to be low. \u00a0<sup>\u00a0<\/sup>The rate of minor hypoglycemia was 0.29 episodes per patient-year in the patients who added Levemir\u00a0to metformin and Victoza. This therapeutic strategy of introducing a GLP-1 receptor agonist prior to initiating exogenous basal insulin appears to be a safe approach. Victoza\u00a0works by stimulating the beta cells to release insulin only when blood sugar levels are high. Due to this glucose-dependent mechanism of action, Victoza<sup>\u00a0<\/sup>is associated with a low rate of hypoglycemia.<\/p>\n<p style=\"text-align: justify;\"><em><strong>Victoza<\/strong><sup>\u00a0<\/sup><strong>has shown weight loss in patients with type 2 diabetes. \u00a0When taken in combination with insulin, do patients still lose weight?<\/strong><\/em><\/p>\n<p style=\"text-align: justify;\">Based on the study of Victoza\u00a0in combination with Levemir, the results showed that the weight loss seen with Victoza was maintained in the patients who added on Levemir. Although not a weight loss product, other clinical studies have found that Victoza\u00a0may help patients lose weight.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Victoza<\/em><\/strong><em><strong>\u00a0and insulin require separate injections, a big drawback for patients. \u00a0Is there work being done to create an all-in-one?<\/strong><\/em><\/p>\n<p style=\"text-align: justify;\">As noted on our corporate web site, Novo Nordisk has an ongoing phase 3 clinical development program testing the safety and efficacy of a fixed ratio combination of liraglutide, the active ingredient in Victoza, and insulin degludec, a basal insulin analog.<\/p>\n<p style=\"text-align: justify;\"><em><strong>What sort of changes do you expect to see coming out of this newly approved treatment regimen? \u00a0<\/strong><\/em><\/p>\n<p style=\"text-align: justify;\">Novo Nordisk cannot speculate on physicians\u2019 prescribing behaviors, but the expanded label means that more patients struggling with blood sugar control at different stages of the disease may benefit from Victoza.<\/p>\n<p style=\"text-align: justify;\">The label now includes data that demonstrate that Victoza<sup>\u00a0<\/sup>may help patients as an add-on therapy to metformin early in the treatment paradigm as well as later, in combination with other oral agents and basal insulin.
